What is Direct Primary Care?
   
    Direct Primary Care is a model of health care delivery that removes the middle man from between the patient and the doctor. Instead of paying copays and deductibles to see your primary care physician, you pay one flat monthly fee, like you would for a gym membership. 
     Most of us have grown accustomed to using insurance to pay for all of our healthcare, even routine visits for primary care. This is one reason insurance has become very expensive. In an effort to curb that expense, the insurance-based system has evolved from paying the physician for each visit, to one that pays the physician for collecting and submitting data, such as blood pressure readings, for their patient population. The insurance industry calls this "quality" based payments. In reality, this undermines the quality of care by forcing the physician to devote the limited time he has with the patient to collecting the many data points the insurance companies require. If you've ever had a doctor's visit where it felt like the doctor was checking off boxes, then you've experienced the effects of insurance based quality care.
       In Direct Primary Care quality means a visit focused on you, not on data collection.

It is important to understand that Direct Primary Care is NOT insurance. It is simply a payment agreement between you and your doctor for services. You should still have insurance to cover unexpected expenses such as hospitalizations, emergency room visits, surgeries and diagnostic testing.

  • Prevention Care
  • Annual, personalized, in-depth, wellness examination and evaluation
  • Sports Physicals
  • School Physicals
  • Scouting physicals
  • Pre-operative evaluations
  • Well-Woman Care
  • Well-Child Care
  • Electrocardiogram (EKG)
  • Chronic disease management (e.g. diabetes, high blood pressure, asthma)
  • Urinalysis
  • Rapid  Strep Test
  • Urine pregnancy testing
  • Removal of skin tags
  • Removal of Cerumen (ear wax)
  • Abscess Incision and Drainage
  • Basic Vision Testing
  • Telemedicine visits
  • Health coaching
  • Minor wound care

Member is responsible for all fees associated with any laboratory testing and specimen analysis, such as pap smear analysis or other testing sent to outside testing centers.

 Non-Medical, Personalized Services:

  After-Hours Access. -direct telephone access to the Physician for guidance regarding urgent concerns that arise unexpectedly after hours.

 Email Access.  You will be given the Physician's email address to which non-urgent communication can be addressed.

  Same Day/Next Day Appointments.  Every reasonable effort shall be made to accommodate same or next day appointments when necessary and requested.

 Specialists Coordination. Coordination of care with specialists to whom you are referred.

OUR DIRECT PRIMARY CARE FEES:

Individual - $75.00 per month

Couples - $140/month

Families - $170/month (2 parents + 1 child ; $20/month each additional child under age 18; max $220/month)

Single parent families - $95/month (1 parent + 1 child; $20/month each additional child under age 18. Max $220/month)

There is an enrollment fee of $99.

 There is a re-enrollment fee of $250 for members who wish to resume membership after terminating it or allow it to lapse.
